This form is used to apply for access to University administration systems. For descriptions of responsibilities, please refer to the Help pages

Training may be required before access is given to some Themis and all Merlin responsibilities. Please view the Themis web site http://www.themis.unimelb.edu.au/support/training.html or the Merlin web site: https://studentadmin-staff.acs.unimelb.edu.au/ for training dates.

Please note that some responsibilities in Environment, Health & Safety incident reporting are not allocated via this form. For more information see: http://www.pb.unimelb.edu.au/ehs/ehs/incidentreporting/themis_resp.php3

Please complete all compulsory fields (indicated with *) and select the responsibilities you require. When the form has been printed and signed by your approver please fax it to the appropriate system administrators:

  • For Themis Access, fax to: 8344 2885
  • For Merlin or Student Admin, fax to IT Operations: 8344 2765
